As many plan providers are combined under one umbrella (such as PHCS) this list is not intended to be a complete list of all plans that we accept. Please feel free to call us if you do not see your specific group listed and we will be glad to check to see if you are covered for massage.
Please check with your plan for limitations and restrictions. Some plans have a limited number of covered appointments.
* MultiPlan includes several hundred different medical providers. Members should see a symbol similar to the ones below on their insurnce card. MultiPlan Members may include: ANC, Group Health, Healthnet, Optum Health, & PacifiCare just to name a few.
 
Clients wishing to use insurance benefits will be required to provide their insurance plan and group or plan number when making the first appointment. Verification of coverage will be REQUIRED prior to services being provided. A copy of the insurance card will be required at the time of the appointment or insurance benefits will not be accepted. Required co-pays must be made at the time of the appointment.
